HDF, HDR, FTR usage

How are the params HDF, HDR, FTR used?

I am looking for a way to include information, such as Javascript, in the html output files without have to include something in each and every xml file. It looks like the open toolkit provides for such, but I can find no documentation for such.

Does any one know of any documentation on modifying the html output using the open toolkit?
